If you lose your health insurance card
Please be careful not to lose your insurance card and beware of theft. The use of an insurance card cannot be suspended even if it was lost or stolen. In the case that you lose your insurance card or it is stolen, Please report it promptly to the police.
Please also promptly carry out procedures through your employer to have the insurance card reissued. If you are a Voluntarily and Continuously Insured Person, please notify this health insurance society.
Procedure
If you lose your insurance card, please fill out the following documents immediately and report to the Health Insurance Union via the personnel department of each company. If you are a voluntary continuing insured person, please attach a copy of your identification card(driver's license, passport, other photo certificate issued by the government agency)to the application form and mail it directly to the Health Insurance Union. If it is lost due to theft etc.,please notify the nearest police immediately.
